COST:$15 before May 1, $20 after
CAUSE:All proceeds go to a scholarship for single mothers.
RACE PACKET PICK UP:Friday, May 14 at Paragon's Office (3651 N. 100 E., Suite 275, Provo)

As a registered participant in this race, I waive any and all claims for myself and my participating minor dependents against officials, volunteers, sponsors, and any other related parties of the Live Your Dream 5K Run/Walk for injury or illness, which may directly or indirectly, result from participation in this race. If any injury or illnesses occur at the time of the race, I take full responsibility and will not place blame on Paragon Wealth Management. I further state that I am registering in proper physical conditions to participate in this event.
